More than a month after her death, the cause of Hayden Panettiere’s passing has been disclosed. The actress, known for her roles in “Heroes,” “Remember the Titans,” and “Ice Princess,” died on August 16, 2026, in Greenville, South Carolina, at the age of 36.
The Greenville County Coroner’s Office confirmed on September 22 that her death was due to the toxic effects of multiple substances, including fentanyl, 4-ANPP, alprazolam, methocarbamol, and quetiapine. The manner of her death was classified as accidental. Fentanyl is a synthetic opioid commonly used for pain relief, while 4-ANPP is a compound linked to the illegal manufacture of fentanyl. Alprazolam, known as Xanax, is a medication prescribed for anxiety, and methocarbamol, sold as Robaxin, is a muscle relaxant. Quetiapine, marketed as Seroquel, is an atypical antipsychotic used to stabilize mood.

According to a statement from the coroner’s office, a 911 call reporting a cardiac arrest was received around 1:51 PM on the day of her death. Emergency responders arrived and found a woman in cardiac arrest. Despite efforts to revive her, she was declared dead at 2:32 PM.
The coroner’s office stated that they worked alongside the Greenville City Police Department during their investigation. Hayden Panettiere was identified as the deceased individual. She was found unresponsive in the residence where she had been staying temporarily. An autopsy performed by a certified forensic pathologist revealed no signs of trauma that could have caused her death.
On August 18, her body was released to her family and transferred to a mortuary that night. At the time of her passing, her boyfriend, Brian Hickerson, and his brother, Zach Hickerson, were present. A police report indicated that Brian showed authorities a bag of medication that Panettiere had been using. The report also described Zach as appearing very emotional.
Panettiere is survived by her daughter, Kaya, who was born in 2014 with her former fiancé, Wladimir Klitschko. The couple raised Kaya together for four years, but in 2018, Panettiere chose to give Klitschko full custody, a decision she described as “incredibly difficult.” She later spoke about this choice on Jay Shetty’s podcast, clarifying that it was not a matter of simply giving away her child, as she had actively sought treatment for her issues at that time.
